How to Enter

Simply complete this form and deposit it at the point of promotion.

 Eligibility

• Eligibility to win is limited to residents of Ohio who are at least 18 years of age.

• Multiple entries and unreadable entry forms will be disqualified.

• Current employees of Great Lakes Water Treatment, Inc. and/or its affiliated entities and their families are not eligible to participate in this promotion.

• This promotion is only valid in the state of Ohio.

Prizes and Conditions

• Awarded gifts or prizes may not be exchanged for cash unless otherwise noted.

• One Grand Prize will be awarded in the form of a $500 The Home Depot® Gift Card.

• Odds of winning are based on number of entries.

• The next drawing will take place on January 15, 2026. 

• After that each drawing will be held before the 15th of the month after the end of each Calendar Quarter: 

• April 15

• July 15

• October 15

• January 15
  for the entries submitted the prior Calendar Quarter.

Winner Notification

• All prize winners will be notified by telephone.

• If Great Lakes Water Treatment Inc. is unable to reach you by telephone, a certified letter will be mailed to the winner requiring a response within 10 business days of the receipt of the letter.

• If no response is received, the prize will be forfeited, and another winner will be drawn.

Publicity and Consent

• Entrants agree to be bound by these Official Promotion Rules.

• Acceptance of any gift or prize constitutes your permission for Great Lakes Water Treatment, Inc. to use your name and likeness for promotional purposes without further compensation, except where prohibited by law.

• Submission of this form authorizes Great Lakes Water Treatment, Inc. and/or its affiliated entities to contact entrants by telephone regarding this promotional drawing and their products and/or services, even if such entrants have registered their telephone number on the National or State Do Not Call List.
